An abnormal origin of a coronary artery is unusual, yet it has clinical implications. Asymptomatic patients to those with angina pectoris, myocardial infarction, syncope, arrhythmias, and sudden cardiac death are all affected. We present a rare case of an abnormal RCA origin with a malignant path between the aorta and the pulmonary artery. Angiography is intrusive and has a poor incidence of cannulation success. As a result, CT coronary angiography is the optimum imaging modality. Patients with aberrant RCAs are more likely to die suddenly and without warning, hence surgical correction is indicated.
