Chronic Osteomyelitis is a debilitating infection that causes serious impairment in the paediatric population due to its late onset. Fibular osteomyelitis is a rare condition, with just a few known cases around the world.

Case Presentation: We present the case of a 9-year-old boy who presented with left leg pain and was later diagnosed with chronic pandiaphyseal osteomyelitis of the fibula. He was treated with En bloc resection and Methicillin-resistant Staphylococcus aureus with culture-guided antibiotic therapy. After two weeks of follow-up, the left leg was fully functional, with typical infection markers after four weeks. Conclusion: Diffuse osteomyelitis of the long bones is difficult to treat since the gold standard of sequestrectomy and debridement will cause limb dysfunction. Chronic fibular osteomyelitis can be treated similarly to a fibula tumour, with en bloc resection providing a successful functional outcome. After four weeks, check for normal infection markers.
